At most large professional web shops and in-house web teams for large
companies, those tasks are generally handled by different people. In
many cases, there is a different person who does the graphics, another
who does the front end (HTML, CSS), and another who does the backend
programming. The larger the company, the more people and the more
distributed the tasks. I've seen cases where the interface design
team was 2000 miles away from the team handling the backend.
