I've got a program that uses UNIX sockets to communicate with other
instances of the same program (screen, if you want to know) and I'm
wondering about "forwarding" the UNIX sockets over an ssh connection.
Is this possible? Is there some trick I can use?

Basically I want to run half of it on one system, and the other half on
another. Any help is appreciated.

You can use port forwarding.
man ssh.
-L option
