I have an application that ssh's from one box to another to create
users and setup secure connections in order for people to take over
customer's desktop with vnc.  I took a look today and noticed that
server A when trying to connect to server B vis ssh was failing and it
looks like it lost the fingerprint of Server B.  I know it should store
it after you click Yes the first time in ~/.ssh/known_hosts, .  Any
idea why it would of lost this?

Change of home directory ~
Change in file ~/.ssh/known_hosts
Change in client s/w storing known hostkeys differently
Server B has a new name or IP
Server B has changed hostkey (manifestation of this depends on your config)
Cosmic rays
