A while back I bought a book in PDF format that requires me to enter my
email address each time the file is opened (to enhance copyright
protection). [1]

Clearly this was generated dynamically after placing the order, and then a
download link sent (which expires).

I am being asked to help someone sell an existing 500 page manual (which
will be a PDF soon), but the chap obviously wants at least a basic level of
security to prevent free and illegal redistribution of his copyright.

My question is:

Would the entire PDF[1 above] have been "built on the fly", *including* the
email security at the time of order, or would that level of security have
somehow been added to the existing PDF file?

A simple example would be helpful.

I am not sure you  can do this with PHP only.
If you have no problem to use an external program, check out pdf
toolkit here: /

You can use the encryption option.
