- New error for me, can't figure it out.

What is causing it:
$error['no_hash'] = "We could not find a current validation request in
our system.<br>Ensure the link used was input correctly.";
If I comment out this line the error goes away.
All I am doing is setting an error message in an array for later retrieval.
What am I not seeing?
