I have a form that people fill out with 6 fields (name, email, phone #,
etc.). One of the fields is a 6 digit code that they have received by email,
(one of 4 possible codes, named "code" inside the PHP). Is it possible to
send the form results via an email to that "code"

I know I can send it to a predetermined address. My question is, is it
possible to get the code from the submission and place it in an email

It's easy to do, but why are you putting your clients to so much
trouble?  Why not give them a link in the email which has a query
string?  Use this to set the return address.
