We do not recommend using a threaded MPM in production with Apache 2.
Use the prefork MPM instead, or use Apache 1. For information on why,
read the related FAQ entry on using Apache2 with a threaded MPM
This warning is a little bit old, is it still true in today PHP/Apache

No - problem is knowing what extensions are thread-safe (but I'd
expect the newer ones / more popular ones to be OK) but if you're
running on Linux it doesn't really impact performance greatly. OTOH if
you're on Solaris running a high volume site then it'd worth chasing
up and reviewing the source code for non-thread safe functions.
