TXT_DB error from openssl in Fedora Core 3

Today I tried to sign a new X.509 certificate using openssl ca on my
Fedora Core 3 machine.  I got an error:
    failed to update database
    TXT_DB error number 2
and no certificate was produced.

Have others seen this problem?  I don't do this very often, so it might
have been going on for a while.  I notice that I got an update to
openssl-0.9.7a-42.2 on October 14; perhaps this is related.

Anyone know what the error message is trying to tell me, or where I can
look it up?  Any suggestions or workarounds welcome.  Thanks.

Sorry for the trouble -- I found it.  This error means that there's
already an entry in the data base with the same distinguished name.  The
old one has be revoked first if you're going to replace it.
