This happens to me if I use ANY anti-spyware app that purports to have
successfully cleaned out the temp directory. Oddly enough, once I get this
error and I delete everything left in the temp directory that isn't in use,
all the add-in buttons come back.

Weird.

Could be the temp directory, I have now narrowed the problem down to NSW06
which does have a disk cleanup program, Which may be the problem. Thanks
