This appears to happen if you maximize the main browser window, and then
switch to another tab. When you do that the browser goes back to "normal"
window mode although it stays sized as if it was maximized. If you now
minimize, shrink, or move the browser window while still on a tab other than
the one you originally maximized the window with, a ghost window shows up,
which you cannot close, move, or minimize. To clear the problem go back to
the original tab whose ghost window is being displayed (it won't show as
maximized anymore even though it's full size). After you have viewed that
window in the tab again the ghost window disappears.
It took me a while to figure out the behavior, but I think what I just wrote
is accurate. I've just stopped maximizing my browser and just left it
occupying the while screen in normal window mode as a way to circumvent this
problem. I've also noticed that if you hit F11 to do view a tabbed page
across the full screen, when you hit F11 again to return to normal mode, all
the other tabs are gone.
