Enterprise CA server dead.

One of our servers that hosted CA services was forcibly removed from
the domain. No backup is available.
I need to install a new Enterprise CA. Can I use the same name as the
old one? What would be the ramifications to services/users if I were
to install a new one with the same name? Is it better to install a new
CA with a different name?

You can remove trails of the previous CA:

http://support.microsoft.com/kb/889250
http://support.microsoft.com/kb/555151

Then install new one. Same name will be possible. Old certificates will
become invalid and will have to be reissued.

Plan for backup and server recovery for the new infrastructure.
