ActiveSync 4.5 (was happening with 4.2 too) installed on Windows XP
PRofessional
Microsoft Outlook 2002.
Exchange

When you go to select itmes to sync, "Notes" is missing from the list.
Where is it?

There's Inbox, Contacts, Calendar, To-Do, etc. Listed under both Exchange
and Windows PC, but there's no Notes under Windows PC as it should be.

What happened to it?

Are you syncing a Smartphone? Don't believe Notes are support on Smartphones
as they do not have a Notes app.

It's a Motorola Q - will it support notes? My client said that she had notes
on the phone, but there was no sync "conduit" appearing in ActiveSync for
Notes.

A Motorola Q is actually a Smartphone, or what MS calls Windows Mobile
Standard today. It does not include a notes application from MS and
ActiveSync won't sync Outlook notes to it. I expect the decision was based
on the dificulty of entering notes on a Smartphone, which was made before
Smartphone OEMs started putting keyboards on the things.

If your client had notes on the phone, it is possible that Motorola added a
notes app themselves, and didn't provide an AS conduit. There are third
party apps that add notes capability to the phone and a conduit to Outlook
notes, such as SmartphoneNotes (clever name, huh), and PhatNotes by
Phatware.

My WM 5/6 smartphones do have Voice Notes, essentially a voice recorder, but
I don't recall these syncing.

You might see what Notes application your client has.and go from there.

Yes, it's a Motorola Q (original Windows Mobile 5).
