First, make sure the file is actually called favicon.ico . IE<7
specifically looks for that file name, and will create a 404 in your
logs if you have eg, example.ico.

Even with the correct file name, there is no way other than putting a
gun to the user's head, to have the favicon show up without the user
bookmarking it. Additionally, if the user clears the cache, those icons
can be lost.
